CASE STUDY

What is a webfolio?

You were introduced to webfolios as a tool in Pebble+ in your first activity. For this assessment you get to build a more comprehensive webfolio. Webfolios can contain a number of pages which you can add, edit or delete at any time.


In this assessment you develop a summary case study. You present a description of a person with a chronic condition. You should write this as a scenario. Your scenario shouldn’t be about a real person, although you may want to draw inspiration from someone you have encountered.

The great thing about doing this as a webfolio is that you can link to resources and can use any of the assets that you have created.

In preparing your case study you need to provide the following:

1. Brief background – description of the case ie a person with a chronic condition with a very brief description of that condition

We provide the following example to give you inspiration:

Peter is a 40 year old builder for a commercial building company overseeing large construction projects. His job involves using a variety of materials and heavy lifting, and he commonly works twelve hour days. He has worked in the same industry since beginning his apprenticeship at age 15.

He does not have income protection insurance and he is the sole income earner for his family that includes his wife Paulette, and four children aged 6, 8, 10 and 13. His children are all in school. The oldest is in a private secondary college, the younger three in a public primary school. One of the primary aged children receives private tutoring due to a learning disability. He and his wife are paying a mortgage and his wife undertakes 15 hours of unpaid volunteer work each week while the children are at school.

The last time Peter went to the doctor was 2 years ago for a work related injury that required a tetanus injection. At this time, Peter’s GP ran some routine tests as Peter had not had a general check up in a long time. These tests showed that Peter had high blood cholesterol, high blood pressure and had pre diabetic blood sugar levels. These results were not followed up.

Peter is reluctant to access health services and does not have a high opinion of doctors. He has suffered from chronic lower back pain for the past 15 years. The back pain is not attributed to any one event and Peter considers that it is “just part of the job”. He is a heavy smoker – 30 a day habit, he consumes at least three stubbies of beer per day and his diet at work is poor. He is overweight – BMI over 30. Peter is frequently short of breath which he dismisses.

2. Health promotion activities and risk factors applicable for that person

3. Brief description of the likely barriers and enablers for effective self management that relate to this person

4. An outline of a self management care plan that would be appropriate for the person. This plan would include identification of the multidisciplinary team that would be appropriate, and the self management tools and interventions that could be used to support the person to self manage.

You can keep updating your webfolio as you go so if you want to get started early you could get your scenario ready and as you work through the subject build up your webfolio as you develop knowledge in the area of chronic condition self management.

You will note that your part 2 assessment is not due until June but you can start working on your webfolio whenever you want.

By the time you have finished your webfolio you will have developed up a fantastic piece of work that will showcase what you have learnt in this area. Remember that the great thing about using Pebble+ is that the webfolio you develop (and other assets) will be available to you and can be updated at any time while you are a student. After you finish studying, the PebblePad company allows free access for one year. After that time you can access your account for a small yearly fee. Your case study may be something that you want to present in the future to demonstrate your commitment to learning, and illustrate your knowledge and skill to a potential employer.

Part 1) Sensory Deprivation Exercise
I would like you to experience what it is like for people (particularly the elderly) to lose some of their sensory ability. It is a sensory deprivation exercise and, if possible, try and have someone help you with it. Be careful of the things you do while doing this exercise as your senses will not be as sharp as they normally are. DO NOT ENGAGE IN ACTVITIES THAT PUT YOU OR OTHERS AT RISK (e.g., DRIVING).
To alter your senses…..
Hearing – Place cotton balls in your ears to muffle sounds.
Vision – Place translucent plastic over the goggles (or glasses if you wear them) and secure with rubber bands or tape. Or look through a piece of translucent plastic.
Touch – Put on rubber or thin material gloves.
You can do one sense at a time if you are alone so that you have your other senses to help you. If you have a helper, do all the senses at the same time to get a perspective of what it is like to be elderly.
Try some everyday activities like reading or watching TV. Try picking up eating utensils with the gloves on.The assignment is to describe what the experience is like. What are your thoughts about your senses being altered? What are some of your feelings about it?

1 respond )>> For this week I chose to try and complete our weekly assignment/conference while having my senses impaired. So please, take note there may be a few mistakes hehehehe.
So I have inserted ear plugs, similar to the ones they give you on airplanes, I am using glasses with a vision altering plastic over them and wearing gloves to type.
First off, to listen to and really understand everything in the lecture notes and short videos I watched online. I had to have the volume turned all the way up, to a point where I had my kids come up and ask if everything was ok, and why I was it was so loud.
I found myself moving around in front of my computer screen to try and get a clearer picture of what I was reading and at times trying to make the letters bigger so it would be easier to read. That took a lot of my concentration away and I found I was reading and re reading the same paragraphs over and over. Not to mention trying to find the right setting.
Trying to type without having full sensation of the keyboard and it’s keys finally made me understand my dad a bit more and why he types like he is picking corn from from the keyboard. Not only does he have to take note and make sure he is choosing the right key, he also has impaired sensation of how many keys he has actually pressed. Not an easy task.
Eventually, going back to my normal senses I really understood how many difficulties losing your senses presents. I can see how it must be even harder when you slowly, start losing each one to a point where you don’t even notice until you go to renew your DL and are told you need glasses. But it would be horrific to all of a sudden partially lose all or anyone of our senses. Very interesting exercise.
2 respond) >>>For this week’s experiment was fascinating because it’s something I never considered during daily life for the past 29 years. For the first part of the experiment I spent a day with hockey gloves on to impair the sense of touch. I even got my girlfriend to brush my teeth for me, which is probably one of the most personally invasive activities we do on a daily basis. The simplest of tasks became difficult and small items such as using a key to open a door or turning on the television. My level of frustration rose throughout the day and even the act of petting my 9 month old puppy seemed more work than I wanted to bother. For the second day I spent the entire work day without my glasses on, which makes me border-line legally blind. I was especially surprised at how much more movement my day required, when you’re not able to see anything from a far. From looking at the time on a clock to being unable to determine the face of a co-worker, this we definitely a harsh lesson to learn. The loss of site was probably the more hindering of the two loss of senses, even though I was still able to roughly make out shapes and colors. With more than 1 billion people in the world disabled, as reported just today by the New York Times, it is so great that today the UN is hosting the first every General Assembly meeting to allow disabled individuals to contribute to the world economy. As Americans we encourage people with special needs to overcome barriers, while still much of the world pushes them aside and restricts them from learning their potential. This experiment showed me just how hard it is to work at overcoming a disability involving the use of touch or site. It definitely gave me more respect for those who do it on a daily basis, but just as important allowed me to empathize far more with disabled people. While it wasn’t the most academic task we’ve done, so far this week’s exercise was by far the most enriching for me as a person.

Now that you have explored some preliminary ideas, you can begin to formulate potential research questions and hypotheses – the next step in the research process. Sometimes the most common question or occurrence can be an intriguing topic. For example, consider the question: Is there a relationship between obesity in young children and television viewing? As you learned in your readings, almost any topic can be researched, as long as there are reputable resources available to support the study. This activity helps you to write a research question that you will explore throughout this course.

Chapter 2 of Gravetter and Forzano describes the processing of getting started in research. The first step is to find a topic using the mind map output and some of the common sources described in your textbook, including personal interest, causal observation, practical problems or questions, vague thoughts, reading others work and observations or theoretical based ideas. Considering primary and secondary sources and avoiding the common mistakes in choosing a topic in research, using online databases such as PsycINFO begin the process of conducting a literature search.

Submit the work you have completed for this activity in one document that includes the following:

1. Your mind map graphic pasted into the document.
2. A brief narrative in which you reflect on your brainstorming experience. How did this process affect your thinking about your proposed topic and your process of formulating a question? Step 1: Identify a potential crisis or emergency where you would have to intervene on behalf of a client. For example, a client tells you that she is afraid to go home due to abuse she has been suffering in the home.

Step 2: Identify organizations, shelters, social service agencies, and first responders in your community that could be potential referral sources for this situation.

Step 3: Identify the contact person or department for each of these resources. Be sure to include phone numbers, emails, faxes, and addresses for each.

Step 4: Contact in person or by phone each resource on your list and ask the questions below. Be sure to document each response. Document any important additional information you glean during your information-gathering calls or visits. Be sure to document all of your attempts to reach each agency and organization.

• Is this the best number to call?
• What are the extents of the services offered?
• What are the criteria for clients to receive these services? Who is eligible?
• How does the organization provide continuity of care to clients? What additional services will be available or offered to clients once they become involved with this organization?
• Will you be able to have contact with your clients once they are involved with this service/organization?
• What is the organization’s policy on client follow-up?
• What role do you or your agency play in responding to emergencies?

Step 5: Once you have made contact with at least seven resources, organize a chart of these resources in a hierarchal order so that you will best be able to respond to the emergency situation that you are investigating. You will have to develop a plan of action and put the first step you need to take at the top with each of the next steps in order of how you would carry out the plan of action. You should have at least three tiers of contacts in your Decision Tree.

Step 6: Documentation – be sure to include a place on your spreadsheet or graph where you can include any required follow–up documentation and notes. You may also put this information in a separate document.

“A well-functioning healthcare system is essential to the publics health. An efficient system that works in concert with organized public efforts and leaves adequate financial resources to invest in programs directed at the health of the entire population is a key goal. The population health approach thus needs to pay attention considerable attention to the working of the public and private healthcare system, as well as that of the public health system.” (Riegelman, Public Health 101 ).
Canada has a three tier healthcare system, and the UK healthcare system which has a two tier system. . In the UK, there is the National Health Service, the free, government run one. However, there is also a private system – which individuals pay for , but many believe you get better, and faster, service. Sort of like the difference between a public school, and a exclusive private one. On the hand, Canada has just the one system. It is “universal,” which means everyone has the same access – rich or poor. If the street person shows up at the hospital before the billionaire (or his illness/injury is more serious), he is seen first. Money cannot get you anywhere in the Canadian system, it is the same for all (Sherice Jackson). Agree or Disagree

It seem to me that every country but the United States have some kind of affordable health care system. These health care systems seem to all have the same idea in which they provide health care reguardless if a person could pay or not. Most European countries seem to have the same basis for their health care system where as the government regulates and budgets the system. According to the videos posted on the Baltimore Chronicle video compilation from cable news, countries like the Netherlands, Great Britian, and several other northern countries in Europe have this health care system that is more affordable than that of the United States. Also their doctors are not paid as must. I like that the Japanese health system is low cost to citizens, the same health care, and the same prices and fees across the board for all the citizens. I would prefer to have the Japanese health care system. They cost half of what American pay and at times you don’t need an appointment and child under 15 are free in Tokyo ( Kiara Crider). I cannot imagine that that many agencies could always play nice with each other on a regular basis. The sheer amount of people employed in the NIH, FDA, and CDC is staggering, not to mention that the agencies have different focuses and interests, except for the good of the American people. The NIH focuses on research, the CDC focuses on getting public health information to the people through all levels of government, and the FDA is the last step towards protecting the public from the researched drugs from the NIH that are promoted by the CDC (Riegelman, 183-184). During times of emergencies like the SARS epidemic in 2003, the agencies can pull together, but collaboration between these agencies does need to be an everyday condition (Riegelman, 186) Linda Swancer. Coordination between agencies are not always the easiest to do because of many limitations and roles. Coordination among public health agencies has been a major challenge that is built into our local, state, and federal system of governance. (Riegelman 186) In recent years, collaborations have become stronger ,and happen more smoothly. When organizations come together it is due to an emergency situation. The blood tests that are usually ordered to diagnose anemias are: hematocrit, hemoglobin, reticulocyte, MCV, MCHC. Hemoglobin is a protein in your red blood cells that carries oxygen to your body’s organs and tissues and transports carbon dioxide from your organs and tissues back to your lungs.
If a hemoglobin test reveals that your hemoglobin level is lower than normal, it means you have a low red blood cell count (anemia). Anemia can have many different causes, including vitamin deficiencies, bleeding and chronic diseases. (1)
A reticulocyte count is a blood test that measures how fast red blood cells called reticulocytes are made by the bone marrow and released into the blood. Reticulocytes are in the blood for about 2 days before developing into mature red blood cells. Normally, about 1% to 2% of the red blood cells in the blood are reticulocytes. (2)
The reticulocyte count rises when there is a lot of blood loss or in certain diseases in which red blood cells are destroyed prematurely, such as hemolytic anemia. Also, being at high altitudes may cause reticulocyte counts to rise, to help you adjust to the lower oxygen levels at high altitudes. (2)
The MCV is used for determining the size of the red blood cells and serves as one of the most valuable measures for classifying anemias. An elevated MCV simply means that your red blood cells (on average) are larger size than normal, and a low MCV means they are smaller size than normal. An elevated or low MCV can mean a variety of things and must be looked at with the rest of the CBC and other lab data. Some Causes of Low MCV are chronic iron deficiency and Microcytic anemia. (3)
A MCHC blood test may be ordered when a person is showing signs of fatigue or weakness, when there is an infection, is bleeding or bruising easily or when there is an inflammation. If MCHC test results are high or elevated, chances are that the person is suffering from macrocytic anemia. Folic acid deficiency, liver disease, hereditary spherocytes, or a vitamin B12 deficiency could be responsible for this kind of anemia. Elevated levels may also appear in burn patients. If MCHC levels are low, the person may be suffering from iron deficiency anemia or thalassemia. (4) ( Marnise Clark). Agree or Disagree

One condition a person who has a tendency to bleed is called anemia. Anemia is the decreased number of red blood cells circulating. There also can be a decrease in red blood cell production, or a higher rate of destruction of red blood cells. This could be caused by certain diseases or iron deficiency. Lab tests are the only way to determine the cause of anemia.
Using lab tests, the type of anemia can be classified into macrocytic, normocytic, or microcytic. These test include blood smears, red blood cell counts, hematocrit and hemoglobin. As times are changing, diversity of societies is increasing. As a result, health care organization must adapt to this change and effectively meet the needs of a newly diverse society. Of course, this will cause many challenges! Many cultural backgrounds have characteristics that will need to be addressed and considered in planning for the particular organization. Examples of this would include different concepts of time, appropriate relationship contextualization, language barriers, respect, views of health, and response to a healthcare organization. Specific examples of this would include the Hispanic ideals of personalismo and respeto; the first being a preference for friendly and warm relationships, and the latter being a demand for respectful behavior (du Pre). Differences among these traits can cause many difficulties for a health care organization to deal with. Difficulties could include scheduling conflicts for differing time consciences, appropriate physician touch or doctor-patient relationship qualifications, language accommodation, addressing a patient, how to get an individual to disclose health information, and other influences on medicine than the traditional approach – faith-based healers, Traditional Chinese Medicine, folk medicine, etc. Solutions to these challenges would be to thoroughly research applicable cultures and to apply that information to health care practice. Examples would be to find translators for language barriers, encourage alternative medicines that are not harmful, even if not helpful, and to find new ways of communicating pharmaceutical instructions that can be easily understood and followed by patients. These solutions should be applied to all applicable cultures in the health care organizations, regardless of background. Cultural background should be factored into every interaction to best accommodate for that culture in the organization (Shelbey Oren). Agree or Disagree

The main differences between Western and Eastern cultures in a healthcare setting originate with the concept of locus of control. According to the text, many Western cultures have an internal locus of control (DuPre, 2013). Many North Americans have moral, religious, and social beliefs that lead them to feel as if they have control over the outcome of situations. As a result of Western internal locus of control, many Americans possess high health-efficiency (DuPre, 2013). Western patients are more interested in new healthcare innovations and methods to improve health. For example, the diet and exercise industry is a corporate giant in the United States as Americans hope to improve themselves and maintain good health. The text also suggests that because Westerners believe in control over health outcomes, that Western views of health conditions and behaviors are shaped by high health self-efficacy. For example, people who are overweight are often considered lazy because it is assumed they have control over their lifestyle and physical wellbeing (DuPre, 2013). Additionally, Western views on death and other serious health outcomes tend to be negative. Naturally, Americans fear death and chronic illness and often go to extreme measures to prevent such situations.

Eastern cultures tend to have an external locus of control (DuPre, 2013). People with Eastern beliefs are usually labeled as fatalistic in that they believe outside sources ultimately determine their fate (DuPre, 2013). External locus of control is associated with low health self-efficacy. The text explains that people with low self-efficacy may be hesitant to seek medical attention or take control of their health outcomes because they believe that their physical state is a result of God’s will or fate (DuPre, 2013). Ultimately, Eastern cultures do not place such an emphasis on a particular body image that may result from lifestyle choices. In some cultures, even death is taken with a sense of acceptance. For example, the Japanese view death as harmonious and dignifying (DuPre, 2013).

Though Western and Eastern views toward healthcare seem to be polar opposite, the triumphs and struggles of medical conditions are universal. The care of elderly adults is an emerging global healthcare issue. The cost of healthcare continues to increase and many families, both Western and Eastern, have felt the negative effects of medical bills and elderly care costs. Furthermore, caregiver consideration and effective communication are traits within the healthcare community that can be appreciated all over the world. Hospitals and clinics should require that employees be educated on a number of cultural values in order to ensure caregivers’ ability to communicate with a wide range of patients ( Morgan Lawrence). New Student





You are the head teacher for the four year old classroom at the Little Angels Learning Center. A new student, Luke, is entering your classroom tomorrow. You meet with your director who fills you in on Luke’s history, and well as challenges he has. He explains the following:

Luke was removed from his single-mother’s care when he was six months old due to neglect. Reportedly, she was propping him up on a pillow to feed him, and leaving him alone in their apartment for hours at a time. When child protection officers became involved, they were able to place Luke with his grandmother. Luke seemed to do well in her home where he was offered nurturance, nourishment, and love. Sadly, Luke’s grandmother passed away when he was 18 months old. Luke was placed with his aunt, who decided after a few months that he required too much attention. He lived with several other relatives before being placed in a foster home six months ago. You learn that this is also Luke’s fourth pre-school setting.

Luke’s foster mother has expressed concern over Luke’s social development. She reports that Luke often isolates himself from the family, and refuses to follow through on any expectations, including brushing his teeth or reading books with their older daughter. When Luke does join the group, he quickly becomes aggressive and tries to take the toys away from their two year old son. He also would prefer to watch television than play with his toys or participate in outdoor activities.

Luke’s foster family wants to remain committed to him. They add that at times, he will hug them or say he loves them, which gives them hope that he has the ability to give and accept affection. They have learned that they can’t “push” him, but allow him to trust at his own pace.

Nutrition Analysis





This is based on a 3 day nutrition food analysis. The uploaded materials are the reports that have already been provided to you. Analyze the report and answer the answers below.



1. Write a paper that answers the following 4 questions (a-d) in essay format using these headings:
a. Factors Affecting What I Eat
i. What factors affect the foods you eat (i.e. convenience, cost, religion, etc…)? Were these factors surprising to you, i.e. were you aware that these factors impact your food choices? Make sure to discuss the factors you reported in your Food Record.
b. My Diet vs. SuperTracker’s Targets/Allowances
Look at your answers in the Food Groups and Calories Report and the Nutrients Report. How did your average intake for the 3 days compare to the recommendations? Were you surprised at how your average actual consumption compared to the recommended amounts of foods you should eat from each food group? Why or why not?
c. My Consumption of Calories
Was your calorie intake within 5% of your calorie allowance? If not, explain if you think this is a concern and why or why not. If you think it is a concern, discuss some strategies on how you could come closer to your estimated caloric allowance. For example, do you have excessive amounts of “empty calories?†Discuss a realistic strategy to improve your behavior.
d. My Fiber Consumption
Do you think you have enough fiber in your diet (look at intake of whole grains, whole fruits, whole vegetables, beans/legumes)? What high fiber foods do you eat? If you do not consume enough fiber, are there any higher fiber foods you could add/substitute in your diet that you would be willing to eat? Please explain your answers.
e. Include a short Introduction and Conclusion
